Assessment Types

"Assessment Occasions" (also called "Assessment Types") are terms used within ResiDex and CliniDex to refer to occasions, instances or times when an Assessment of some kind would be performed.

The Admission Assessment, for instance, would be performed upon a client's admission to your facility or program.

Because requirements for Assessments vary by state, county, and organization, ResiDex and CliniDex are flexible to accommodate a wide variety of needs.   CliniDex allows you to establish specific assessment occasions and the content included in each assessment.  Every assessment can be customized to meet your specific needs!

Some common assessment occasions include:

Pre-Admission Assessments

There are at least three reasons that Assisted Living facilities conduct a pre-admission assessment.

  1. Some states  require that a prospective resident be assessed for appropriateness for the facility before being admitted.  Even if not required, a pre-admission screening ensures that the agency or facility will be able to meet the needs of the prospective resident.  
  2. Some facilities that provide services to residents with a higher level of acuity need to ensure that all of the necessary equipment or supplies required to serve the client are provided.
  3. Preadmission assessments that flow into the admission assessment can ease the workload upon admission.

Admission Assessments

The admission assessment is used to document the client's condition and determine the client's current needs for service. Among other issues addressed, the admission assessment includes questions about the clients need for services relating to:

  • Activities of Daily Living (ADLs)
  • Independent Living
  • Medications used
  • Physical status
  • Orientation and cognition
  • Safety.

As with the other assessment occasions, you can customize the assessment for your campus to include or exclude questions to fit the needs of your campus.

Clinical Update 

(Performed quarterly, annually, due to significant change of condition, etc...)

The Clinical Update is a follow-up to the Admission Assessment. This assessment is typically performed at whatever frequency is required by law and/or when the nurse feels it appropriate. The approach used in CliniDex gives you visibility to the answers provided in the admission assessment or the previous clinical update;  the Clinical Update is a review of previously answered items and requires the assessor only to update those that have changed. 

As with the other assessment occasions, you can customize the assessment for your campus to include or exclude questions to fit the needs of your campus.
