Complex Med Orders / Steroid Tapers

Complex medication orders such as steroid tapers can be confusing- with varying doses offered at varying intervals.  This document will walk the nurse through entry of a steroid taper in RTasks.

Example:

Resident Harriett B. Was prescribed Methylprednisone 4m in a tapering schedule:

  • Day 1: 
    • 8am - 2 tabs
    • 12N - 1 tab
    • 6pm - 1 tab
    • 8pm - 2 tabs 
  • Day 2:
    • 8am - 1 tab
    • 12N - 1 tab
    • 6pm - 1 tab
    • 8pm - 2 tabs
  • Day 3:  
    • 8am - 1 tab
    • 12PM - 1 tab
    • 6pm - 1 tab
    • 8pm -  1 tab
  • Day 4:
    • 8am - 1 tab
    • 12N - 1 tab
    • 8pm - 1 tab
  • Day 5:
    • 8am - 1 tab
    • 6pm - 1 tab
  • Day 6:
    • 8am - 1 tab

Entry into RTasks:  

The first entry covered the 2mg doses for Day 1 and was entered as follows.  Note, we opted to get detailed in our instructions, feeling that this would serve as a visual reference.  

We started the med at 12am the date before, and ended it an hour or two after the 2nd dose was to be given.  We essentially created a window during which the med could be given at the specified time

Next, we created a new Methylprednisone entry to document the 1 tab 12N and 6pm doses (shown in blue)

Then we addressed the 8am, 1 tablet doses (in green, below)

Again, we created that "window" using the start and stop dates.

We continued to create Methylprednisone entries for the doses shown in purple, yellow, and pink

Once entry was complete, we navigated to Fax/Print > Reports > Paper MAR category > Med Sheets to view a paper MAR;  this allowed us to cross-check the med entry and review the schedule for accuracy.
